Song switch
The song used here was originally "This must be the place (Naive melody)" by Talking Heads -- sorry, I can't remember the source for this. It's a song about not being sure where your home is or what is right for you, and would have tied well into Juliet's ambivalence towards her island life; Outwardly she desperately wants to go home, but she also has ties to the Others, and probably not much to go home to.
It was replaced at the last minute with "Downtown", which was reputedly also the original nickname of the barracks untill fans started referring to the place as Otherville. She smiles at the irony of the lyrics, about "downtown" being the place to find hope and companionship when you're lost: She doesn't want to be there, but is in fact less lonely than she was off the island.
The song was used in the movie "Girl, Interrupted" which is set in a mental hospital. Juliet has some resemblance to "Lisa Rowe"

Double meaning of song
The song, while obviously emotional because it was the last song Juliet heard with her sister, is also used because she 'is alone and life is making her lonely' but can't go downtown.
